CLASS TITLE: TRANSITION COACH

 

This position is for the 2024/2025 school year.

 

BASIC FUNCTION: To help effectively guide students through academic and career exploration activities promoting elementary, middle, high school and postsecondary readiness.

 

REPRESENTATIVE DUTIES/ESSENTIAL FUNCTIONS:

  • Work with the administration, staff, and faculty of elementary, middle and high schools to promote RCS college and career pathways and to define the college and career awareness process, providing each grade level with appropriate transition readiness guidance.
  • Ensure the provision of intensive programs to low-achieving students to prepare them to take advanced classes.
  • Track and monitor student progress in attaining transition readiness benchmarks for academic and/or career readiness
  • Facilitate the process of having low-achieving students participate successfully in coursework aligned with transition readiness standards.
  • Develop student and family understanding of college and career options as well as pathways to high school and postsecondary success.
  • Coordinate and facilitate presentations, student workshops, parent meetings, and college and career awareness activities for each grade level that promote HCS college and career pathways, postsecondary, and career options.
  • Work with CTE teachers to ensure students are transition ready.
  • Work with classroom teachers to ensure they target low-achieving students to ensure they are transition ready.
  • Serve as a resource for guidance on ILP development, beginning in the middle school programs.

 

College & Career Coach

 

  • Collect and examine data to identify and monitor individual student needs, gaps, and progress related to    transition readiness.
  • Provide assistance and training to school staff in design and delivery of programs and events to address student achievement related to transition readiness such as Close the Deal, Career Fairs, and Work Ethic Certification.
  • Assist with designing and delivering summer transition programs such as Freshman Orientation and Freshman parent night for low-achieving students.
  • Assist with and attend Parent and Family Engagement events relative to transition readiness.
  • Coordinate connections between schools and parents/families/community members to facilitate academic and career readiness.
  • Communicate directly with parents of students who have not attained transition readiness benchmarks.
  • Promotion of the program and its initiatives.
  • Attend relevant meetings and training related to job duties.
  • Gather and disseminate information about innovative programming.
  • Remain current with educational developments and requirements related to transition readiness.
  • Complete other duties as assigned by the supervisor.
  • Assist with Career Pathway implementation at all levels.
  • Assist with Preschool to Kindergarten transitioning.
  • Assist with Kindergarten to 1st Grade transitioning.
  • Assist with Elementary to Middle transitioning.
  • Meet with parents in a 1:1 setting for middle to high school transitioning.
  • Complete 1:1 scheduling in high schools.
  • Assist with implementation and curriculum mapping of standards at all levels.

 

EDUCATION AND EXPERIENCE:  Valid Kentucky Teacher Certification for teaching in grades 9-12.

 

EMPLOYMENT TERMS:  187 days 

 

EVALUATION: Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Certified Personnel.
